2012 Photography Club Membership Form

By submitting this form to apply for or renew your membership in The Photography Club of Sun City Hilton Head (PCSCHH) you are agreeing to have the PCSCHH dues charged to your Sun City Community Association account for which you will be billed at the end of the month. Membership dues are $20 for the first household member and $10 for an additional member in the same household. There is no extra charge for this service.

Badge Name Your first name preferance for the badge.

House Number *

Street Name * The name your street without the house number.

Telephone Number Your ten digit telephone number. (example format: xxx-xxx-xxxx)

Email Address Email address you want the club to use to send you stuff.

Community Association Membership Number * As it appears on your current SCHH Community Association Card.

Household Membership/s * Indicate if you are the first or an additional member in the same household. NOTE: Only ONE member per form.
